How to apply?

You can apply using the online form or by filling out this application form and sending it by email to info@hemofilici.cz. The Council of the Society will then approve your membership at its next meeting. Here you can find an online form for any changes to your personal details.

Membership fees

The amount is voluntary, but the annual minimum is: CZK 500 for individuals; CZK 1,000 for legal entities.

The fee can be paid by bank transfer to the Czech Hemophilia Society account 2109200873/2700. The membership fee must be paid by June 30 of the given calendar year. For correct identification of the payment, please provide the correct variable symbol - your membership number. Alternatively, it is sufficient to include the member's name and purpose of payment in the message for the recipient (e.g. Jan Novak - membership fee). This must be observed, otherwise the payment cannot be identified.

Membership in the Czech Hemophilia Society is governed by the CSH statutes.
